As a new client, Female & Fit asks you to ideally read and complete the following documents in detail so that your program is individualized specifically to meet your goals.
This document includes:
- Coaching Agreement
- Comprehensive Client Information Sheet
- Initial Body Composition Assessment
- Medical History and Present Medical Condition Questionnaire
- Goal Sheet
- 4 Pictures (front, left side, right side, back views)
Note: all these documents are located on one response form that will be sent to you once you invest in Female & Fit. Please remember to click on "submit". I will also need for you to take four pictures of yourself with or without your head. Please date these for a baseline reference. The positions will be a front pose, left side pose, right side pose and a back pose. In order to maintain modesty, please take pictures in shorts or a bathing suit. Females make sure your core area is showing. It is best to take pictures in a bikini and send these pictures via email to femaleandfit@gmail.com.
